Job Duties and Responsibilities:

Assessing patients to evaluate their physical function and limitations within their home environment. Developing individualized treatment plans based on the patient's specific needs and goals. Implementing therapeutic interventions to improve mobility, strength, balance, and pain management. Educating patients and caregivers on diagnosis, safety precautions, and home exercise programs. Collaborating with an interdisciplinary team to ensure comprehensive care and optimal patient outcomes.

Minimum Education and Requirements:

Active License to practice

Education:

Completion of an accredited Physical Therapy license, relevant education, and state licensure Current CPR Certification Valid driver's license Social Security card Recent TB Results Relevant experience preferred Must be 21 years of age or older strong motivation and commitment to delivering high quality-patient care Pay Depends on Experience and will be discussed with applicant at the time of the interview by employer.
