Full-Time and/or PRN Physical Therapist (PT) - In-House Rehab | Sherman, TX Join Our In-House Therapy Team at Focused Care at Sherman! Position Overview As a Physical Therapist (PT), you will evaluate, develop, and implement individualized, evidence-based treatment programs designed to improve residents' mobility, strength, balance, endurance, and overall functional independence. You will play a vital role in interdisciplinary collaboration, resident and caregiver education, and achieving exceptional clinical outcomes. Key Responsibilities Evaluate residents to assess mobility limitations, strength deficits, balance impairments, pain, and functional challenges Develop and implement individualized treatment plans focused on mobility, gait training, fall prevention, strengthening, endurance, and pain management Monitor resident progress and modify treatment plans to maximize functional outcomes Supervise and collaborate with Physical Therapy Assistants (PTAs) to ensure high-quality patient care Recommend assistive devices, adaptive equipment, and environmental modifications to promote safety and independence Educate residents, families, and caregivers on therapeutic exercises, mobility strategies, and discharge recommendations Maintain accurate and timely documentation in accordance with state, federal, and company requirements Participate in interdisciplinary team meetings, care plan conferences, and resident-centered care initiatives Ensure compliance with all regulatory standards, professional ethics, and facility policies Qualifications Active Texas Physical Therapist (PT) license In good standing with all applicable licensing boards and regulatory agencies Experience in long-term care, skilled nursing, geriatric rehabilitation, or post-acute care preferred Familiarity with Net Health documentation software is a plus Strong clinical assessment, communication, leadership, and problem-solving skills Passion for helping older adults achieve their highest level of function and independence Why Join FPACP?
