What You'll DoDeliver Patient-Centered CareEvaluate and assess patients' functional status, mobility, strength, and rehabilitation potentialDevelop and implement individualized care plans in collaboration with physicians and the care teamProvide hands-on therapy to improve mobility, strength, balance, and independenceEducate patients and caregivers on treatment plans, exercises, and safety strategiesDrive Outcomes & RecoveryMonitor patient progress and adjust treatment plans as neededSupport safe transitions of care and contribute to discharge planningIdentify additional patient needs and collaborate with interdisciplinary teamsCollaborate & CommunicatePartner closely with nurses, therapists, and physicians to ensure coordinated careDocument patient care and progress accurately and timely within the EMRParticipate in case conferences and team meetingsLead & SupportSupervise Physical Therapist Assistants (PTAs) in accordance with state guidelinesProvide guidance and education to caregivers and support staffWhat You BringGraduate of an accredited Physical Therapy program (DPT or Master's)Current PT license in the state of practiceMinimum 1â2 years of experience (home health experience preferred)Strong clinical assessment and critical thinking skillsExcellent communication and patient education abilitiesCurrent CPR certificationValid driver's license, reliable transportation, and auto insuranceWhat Sets You ApartYou value autonomy and thrive in a field-based environmentYou're passionate about patient independence and functional outcomesYou build strong, trusting relationships with patients and familiesYou bring both clinical excellence and compassionate care to every visitWhat We OfferHealth & WellnessMedical, Dental & Vision coveragePharmacy benefitsVirtual care & mental health support
