Duties Conduct comprehensive assessments of patients' physical conditions, utilizing your expertise in anatomy and physiology to develop tailored care plans. Deliver effective physiotherapy interventions such as geriatrics-focused treatments to promote healing and functional improvement. Manage discharge planning by coordinating with multidisciplinary teams. Document patient progress accurately using electronic medical record (EMR) systems, maintaining detailed records of treatments administered and outcomes achieved. Educate patients and caregivers on injury prevention, home exercises, and community resources to support ongoing recovery outside clinical settings. Stay current with advances in physiotherapy practices, medical terminology, and healthcare regulations to deliver high-quality patient care across inpatient, outpatient, home health, and clinic environments. Qualifications Valid licensure or certification as a licensed Physical Therapist with relevant regulatory bodies. Demonstrated experience in acute care hospitals, outpatient clinics, or home health services with a focus on patient-centered care. Strong knowledge of anatomy, physiology, medical terminology, and physiotherapy techniques. Proven ability to develop comprehensive care plans incorporating discharge planning and community care strategies. Experience working with diverse populations while gaining familiarity with home & community care is highly desirable. Proficiency in EMR systems for documentation and treatment tracking; excellent communication skills for interdisciplinary collaboration. Ability to adapt treatment approaches based on patient needs.
