Physical Therapist Job Summary:
We are seeking a dedicated and compassionate Physical Therapist to join our Long Term Care facility on a part-time basis. The ideal candidate will work with elderly residents to improve their physical function and quality of life through comprehensive rehabilitation services.
Responsibilities and Duties:
1. Conduct thorough evaluations of residents to assess physical functioning and mobility. 2. Develop individualized treatment plans to address residents' specific needs and goals. 3. Implement therapeutic exercises and modalities to improve strength, balance, and coordination. 4. Collaborate with interdisciplinary team members to provide holistic care for residents. 5. Educate residents and their families on safe and effective techniques for maximizing functional independence. 6. Monitor residents' progress and adjust treatment plans as needed to ensure optimal outcomes. 7. Maintain accurate documentation of treatments and progress notes in accordance with regulatory guidelines. 8. Participate in team meetings and contribute to care planning and goal setting for residents.
Qualifications and Skills:
