Job Description
Job Summary We are seeking a dedicated and skilled Physical Therapist to join our team! The ideal candidate will have a strong background in patient care and a passion for helping individuals regain their mobility and improve their quality of life. Our Physical Therapists develop and implement personalized care plans, utilizing evidence-based practices to achieve optimal patient outcomes. Responsibilities Conduct thorough assessments of patients' physical abilities and limitations Develop individualized treatment plans based on patient evaluations and goals Implement therapeutic exercises and interventions to improve strength, flexibility, and mobility Monitor patient progress and modify treatment plans as necessary to ensure effective rehabilitation Collaborate with multidisciplinary teams to coordinate comprehensive patient care Educate patients and their families on exercises, home care techniques, and lifestyle modifications Maintain accurate documentation of patient assessments, treatments, and progress using EMR system Provide discharge planning support to ensure smooth transitions from outpatient care to home or community settings Skills Strong knowledge of physiology and anatomy relevant to physical therapy practice Proficiency in medical terminology related to rehabilitation and patient care Experience in various therapy settings including acute care, outpatient clinics, geriatrics, pediatrics, orthopedics, and home health Familiarity with discharge planning processes and the development of effective care plans Ability to perform trigger point therapy techniques for pain management and hands-on therapy including exercises, stretches, and manual techniques Excellent communication skills for educating patients and collaborating with healthcare professionals Compassionate approach towards patient care with a focus on improving quality of life through rehabilitation efforts New grads are encouraged to apply!
