Physical Therapist

Position Summary We are seeking a licensed Physical Therapist (PT) to join our home health team. In this role, you will provide in-home evaluations, plan and deliver therapeutic interventions, educate patients and caregivers, and collaborate with interdisciplinary team members to optimize functional outcomes and quality of life for our patients. Key Responsibilities Perform comprehensive evaluations, assessments, and re‐evaluations in the patient's home Develop individualized treatment plans, set goals, and monitor progress Deliver hands-on therapy, exercise instruction, gait training, balance, strength, and mobility interventions Educate patients, family members, and caregivers about home exercise programs, safety, fall prevention, and adaptive strategies Document timely, accurate notes in accordance with agency and regulatory requirements Communicate and coordinate with physicians, nursing, occupational therapy, speech therapy, case managers, and other providers Monitor and revise plan of care as needed Ensure compliance with all regulatory, quality, and documentation standards Travel within assigned territory to visit patients (mileage reimbursement or company vehicle, if applicable) Requirements Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program Valid, unrestricted Physical Therapist license in the Commonwealth of Virginia

CPR / BLS

certification Valid driver's license and reliable transportation Excellent communication, organizational, and time management skills Self-motivation, independence, and ability to function with minimal direct supervision Prior home health or outpatient experience preferred (but not mandatory) Comfort working in varied home environments

$60.00 - $80.00 per hour
