Job Description

Physical Therapist (PRN) Touching Hearts at Home of Central VA Louisa, VA 23093 Part-time Part-time Touching Hearts at Home is seeking a compassionate and motivated Physical Therapist (PRN) to join our growing home health team. This position offers flexible scheduling and the opportunity to make a meaningful difference by providing high-quality physical therapy services to patients in the comfort of their homes. Position Summary The PRN Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, and providing skilled physical therapy services that promote mobility, independence, safety, and overall quality of life. The Physical Therapist will work collaboratively with physicians, nurses, caregivers, and other healthcare professionals to ensure optimal patient outcomes. Essential Duties and Responsibilities Conduct comprehensive physical therapy evaluations in patients' homes. Develop and implement individualized treatment plans based on patient needs and physician orders. Provide therapeutic exercises, gait training, balance training, transfer training, and other skilled interventions. Assess patient progress and modify treatment plans as appropriate. Educate patients and caregivers regarding home exercise programs, fall prevention, safety, and disease management. Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements. Communicate effectively with physicians, caregivers, and interdisciplinary team members regarding patient status and progress. Participate in care coordination and case conferences as needed. Ensure compliance with all federal, state, and agency standards for home health services.
Qualifications Required:
Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the state of practice. Current CPR/BLS certification. Valid driver's license, reliable transportation, and proof of auto insurance. Ability to travel within the agency's service area. Strong clinical assessment and documentation skills.
Preferred:
One (1) year or more of experience in home health, outpatient rehabilitation, skilled nursing, or acute care settings. Experience with electronic medical records and point-of-care documentation. Knowledge of Medicare home health regulations and documentation requirements. Physical Requirements Ability to travel between patient homes. Ability to assist patients with mobility, transfers, ambulation, and therapeutic activities. Ability to lift, carry, push, and pull up to 50 pounds as needed. Ability to stand, walk, bend, kneel, and reach throughout the workday. Schedule PRN (as needed) Flexible scheduling based on patient referrals and agency needs.
