At BrightStar Care of Chesapeake/Virginia Beach/Williamsburg, we are committed to providing exceptional care while supporting our team members with a positive and rewarding work environment. We are seeking a dedicated Physical Therapist to provide skilled home health services to patients throughout the Hampton Roads area. The ideal candidate is patient-focused, reliable, and passionate about helping individuals improve mobility, restore function, and enhance their quality of life in the comfort of their own homes. This is a PRN position, allowing you to pick up visits based on your availability and preferred coverage area.
Start Date:
Immediate openings available Responsibilities Evaluate patients and develop individualized treatment plans based on physician orders and patient goals. Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and functional independence. Follow the established Plan of Care and collaborate with physicians and other healthcare professionals to achieve optimal patient outcomes. Accurately document evaluations, treatments, progress notes, and patient outcomes. Communicate regularly with the Director of Nursing and interdisciplinary care team regarding patient progress, concerns, and changes in condition. Educate patients, families, and caregivers on exercises, mobility techniques, fall prevention, and home safety recommendations. Demonstrate patience, compassion, and professionalism while working with patients of varying ages and diagnoses. Maintain compliance with agency policies, HIPAA regulations, Medicare guidelines, and professional practice standards. Participate in case conferences, training, and continuing education as required. Qualifications Current Virginia Physical Therapist (PT) license in good standing. Graduate of an accredited Physical Therapy program. Minimum of one year of clinical experience preferred; home health experience strongly preferred. Must be able to pass a background check with no barrier crimes or theft-related offenses. Must be able to pass a 10-panel drug screen. Experience working with geriatric and rehabilitation patients preferred. Strong clinical assessment and treatment planning skills. Excellent communication, organizational, and interpersonal skills. Comfortable utilizing electronic charting and documentation systems. Current CPR certification. Valid driver's license and reliable transportation. Negative TB test within the past year.
