Job Description

Job Summary The Physical Therapist is responsible for evaluating and treating clients in the home setting to improve mobility, strength, balance, endurance, safety, and functional independence. The Physical Therapist will conduct home safety evaluations, reinforce weight-bearing status and range-of-motion restrictions, provide progressive mobility training with assistive devices as indicated, and implement general conditioning therapies based on the client's care plan and physician orders Duties Conduct physical therapy evaluations to assess mobility, strength, balance, gait, endurance, pain, range of motion, and functional limitations. Perform home safety evaluations and recommend environmental modifications to reduce fall risk and improve safe mobility. Reinforce physician-ordered weight-bearing status, range-of-motion restrictions, and post-operative precautions. Provide progressive mobility training using assistive devices such as walkers, canes, wheelchairs, or other equipment as indicated. Provide therapeutic exercises and general conditioning therapies to support recovery and functional independence. Train clients in safe transfers, ambulation, gait techniques, stair negotiation, and fall-prevention strategies. Educate clients, caregivers, and family members on safe mobility, proper body mechanics, use of assistive devices, and home exercise programs. Monitor client progress and adjust the treatment plan as clinically appropriate. Document evaluations, therapy visits, client response, progress toward goals, and discharge recommendations in accordance with agency and regulatory requirements. Experience Proven experience in acute care hospital settings with familiarity in inpatient and outpatient care environments. Demonstrated ability to develop comprehensive care plans incorporating discharge planning and community care strategies. Prior work in pediatrics, geriatrics, orthopedics, or home & community care preferred. Strong knowledge of physiology, anatomy, medical terminology, and the principles of physiotherapy. Hands-on experience with EMR systems for documentation and patient management. Background working within hospital or clinic settings that involve patient care across diverse populations. Knowledge of trigger point therapy techniques and orthopedic rehabilitation methods is highly desirable. $100.00 - $150.00 per hour
