Physical Therapist

Overview:

We are looking for a committed Physical Therapist to join our Lamoille Home Health & Hospice team full

• time.

These services are home based, professional judgement and a strong knowledge of autonomy is highly valued. The ideal candidate will have a passion for patient care, ability to plan, implement, and coordinate a physical therapy program from eligibility determination to exit. • time (29+ hours)(paid out over 6 months).

Duties:

• Conduct patient assessments and develop individualized treatment plans
• Implement therapeutic exercises and modalities to aid in patient recovery
• Educate patients and their families on treatment plans and home exercises
• Collaborate with other healthcare professionals to ensure comprehensive patient care
• Maintain accurate patient records and documentation

Requirements:

• Degree in Physical Therapy from an accredited program
• State licensure as a Physical Therapist
• Strong knowledge of medical terminology, anatomy, and physiology
• Experience in home health preferred but not required
