• Required (New grads will not be considered for this need) BLS (AHA) Updated CAQH Required in profile NPI required in Profile Must be flexible
• Will need to work Inpatient PT as needed Must be able to treat all normal diagnoses seen in outpatient clinics. Graduate from an accredited school of Physical Therapy
• Required Previous Rehab Experience
• Required Previous travel experience

• REQUIRED

• At least 3 mos.

CPSI EMR

experience

• Highly Preferred Previous experience working with PTA's
• Preferred This is a small community without public transport.

Traveler candidate must have their own vehicle. This is a firm requirement.

Required Qualifications Physical Therapist, Outpatient Experience:

2 years

Licenses:

PT(Compact Accepted)

Certifications:

BLS(Copy Needed)

SSN Required DOB Required References:
