Job Overview:
We are seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health team. In this rewarding role, you will help older adults improve mobility, restore function, manage pain, and maintain independence while providing care in the comfort of their homes. The Physical Therapist is responsible for evaluating and diagnosing patients, developing individualized treatment plans, establishing therapeutic goals, and determining the frequency and duration of care. This position offers the opportunity to work collaboratively with an interdisciplinary team to deliver personalized, patient-centered care.
Responsibilities:
Perform comprehensive patient evaluations and assessments. Develop individualized treatment plans based on patient needs and goals. Assess muscle strength, mobility, gait, range of motion, and rehabilitation potential. Deliver physical therapy services in patients' homes. Evaluate home environments and recommend safety improvements. Educate patients, families, and caregivers on therapy techniques, exercises, and safety practices. Monitor patient progress and modify treatment plans as needed.
Qualifications Required:
Graduate of an accredited Physical Therapy program (DPT or MPT). Current Washington State Physical Therapist License in good standing. 2-5 years of Physical Therapy experience. Current CPR and BLS Certification.
Physical Requirements:
Ability to lift up to 50 pounds. Ability to work in a variety of home care environments. Strong observation and clinical assessment skills. Ability to communicate effectively with patients, caregivers, and healthcare professionals. Reliable, dependable, and committed to delivering high-quality patient care.

Are you comfortable traveling throughout the assigned service area to provide patient care in home settings?
