Job Title:
Physical Therapist (PT) | Home Health (Also known as: Home Health Physical Therapist, Rehabilitation Therapist, PT, Physical Therapy Clinician, Home Care Physical Therapist) 1. Position Overview Seeking a Full-Time Physical Therapist (PT) to join our team.
Schedule:
Day Shift, Monday-Friday 3. Key Responsibilities Evaluate patients and develop individualized treatment plans. Provide physical therapy services in home-based settings. Improve mobility, strength, balance, and functional independence. Educate patients and caregivers on exercises and safety techniques. Monitor patient progress and update care plans as needed. Collaborate with interdisciplinary healthcare teams. Maintain accurate documentation and compliance with care standards. 4. Requirements Active Physical Therapist (PT) license. 2-5 years of experience. Valid driver's license required. Strong communication, organization, and teamwork skills. Ability to work independently in home care settings. 5. Education Graduate of an accredited Physical Therapy program (DPT or MPT). 6. Certifications Current Physical Therapist License. Current CPR Certification. Current BLS Certification. 7. Preferred Qualifications Home Health experience preferred. Hospice experience preferred. Experience with Electronic Medical Records (EMR). Pay:
$111,446.00 - $139,256.00 per year
