$111,446 - $139,256 per year About the Opportunity Wesley is seeking a compassionate and dedicated Physical Therapist (PT) to join our Home Health & Hospice team. In this rewarding role, you will help patients improve mobility, manage pain, maintain independence, and enhance their quality of life while providing care in the comfort of their homes. As a Physical Therapist, you will work collaboratively with an interdisciplinary healthcare team to deliver personalized, patient-centered care that aligns with each individual's goals and needs. Responsibilities Perform comprehensive patient evaluations and assessments. Develop individualized treatment plans and therapeutic goals. Assess mobility, gait, muscle strength, range of motion, and rehabilitation potential. Provide physical therapy services in patients' homes. Educate patients, families, and caregivers on treatment plans and safety techniques. Collaborate with physicians, nurses, social workers, and other healthcare professionals. Monitor patient progress and modify treatment plans as appropriate. Participate in discharge planning and care conferences. Supervise Physical Therapy Assistants (PTAs) in accordance with state regulations. Maintain accurate documentation and compliance with company policies and healthcare regulations. Evaluate home environments for patient safety and recommend modifications when needed. Qualifications Graduate of an accredited Physical Therapy program (DPT or MPT). Active Washington State Physical Therapist License in good standing. 2-5 years of Physical Therapy experience. Current CPR and BLS Certification. Valid Driver's License and reliable transportation. Home Health and/or Hospice experience preferred. Strong communication, organizational, and clinical decision-making skills. Ability to work independently and as part of a multidisciplinary team.
