Position Summary:
We are seeking a dedicated and skilled Physical Therapist to join our skilled nursing facility (SNF) team. In this role, you will provide comprehensive physiotherapy services to patients across diverse age groups, including geriatrics, pediatrics, and post-acute populations.
Key Responsibilities:
Evaluate residents' physical conditions and functional abilities. Develop individualized treatment plans based on patient needs and physician orders. Provide therapeutic exercises, gait training, balance training, and mobility interventions. Assist patients in improving strength, flexibility, endurance, and coordination. Train patients in the use of assistive devices such as walkers, canes, and wheelchairs. Monitor patient progress and modify treatment plans as needed. Educate patients, families, and caregivers on therapy goals and home exercise programs. Maintain accurate and timely documentation of evaluations, treatments, and progress notes. Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals. Ensure compliance with facility policies, state regulations, and Medicare guidelines. Participate in discharge planning and recommend appropriate follow-up care.
