Transitions At Home , is currently seeking to hire a Full-Time, compassionate and experienced Physical Therapist to join our growing home health team. Flexible daytime hours:

Your Role:

As a Home Health Physical Therapist, you'll deliver skilled therapy services based on individualized care plans and physician orders. You'll work closely with the interdisciplinary team to support patients in regaining strength, mobility, and independence at home.

Key Responsibilities:

Complete OASIS assessments, evaluations, and progress notes Develop and revise care plans Perform skilled physical therapy procedures Educate patients and families on home programs and daily activities Collaborate with physicians and the care team Participate in IDT meetings, in-service training, and QA initiatives Ensure accurate documentation and use of ICD-10 codes

Qualifications:
