Pay:

$50•57/hr on W-2, depending on experience. Mileage reimbursement provided (from first patient to last patient visit)

Job Summary:

We are seeking an experienced Travel Physical Therapist (PT) to join a home health team in Indianapolis, IN. This role involves delivering high-quality, patient-centered care in a home setting, performing evaluations, developing care plans, and supporting patient recovery and mobility.

Key Responsibilities:

Perform Start of Care (SOC), Evaluations, Resumption of Care (ROC), and Recertifications Provide ongoing therapy visits and reassess patient progress Develop and implement individualized treatment plans to improve mobility and function Educate patients and caregivers on therapeutic exercises and safety practices Maintain accurate and timely clinical documentation Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes Ensure compliance with home health regulations and agency policies

Required Qualifications:

Active Physical Therapist (PT) license (state-specific) Doctor of Physical Therapy (DPT) required Current CPR certification Prior Home Health experience Strong clinical assessment and patient care skills Ability to manage an independent, field-based workload

Certification:
