Home Health:
Conduct in-home physical therapy assessments and evaluations Develop and implement individualized treatment plans for patients Improve mobility, strength, balance, and functional independence Support recovery for patients with injury, illness, or post-surgical needs Educate patients and caregivers on exercises and ongoing care plans Maintain accurate documentation and progress notes Collaborate with physicians and multidisciplinary care teams Ensure safe delivery of therapy within home environments
Requirements:
BLS certification West Virginia state Physical Therapist license Minimum of 1 year Physical Therapist home health experience
