per visit + mileage reimbursement First Choice Home Health is seeking a compassionate, skilled Physical Therapist to join our growing home health team in the Phoenix and East Valley area. Service Area Phoenix, Chandler, Gilbert, Mesa, Tempe, and surrounding East Valley communities. What You'll Do Deliver skilled physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist (PT) Implement individualized treatment plans focused on improving mobility, strength, balance, endurance, and functional independence Accurately follow the established Plan of Care (POC) and report patient progress, changes, or concerns to the supervising PT Perform and document therapeutic exercises, gait training, transfer training, and use of assistive devices Educate patients and caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques Complete timely, accurate, and compliant documentation in accordance with agency policy, Medicare guidelines, and regulatory standards Travel to patient homes within designated service area and manage daily scheduling efficiently Participate in case conferences, team meetings, and quality improvement initiatives as needed Maintain current licensure, certifications, and competency requirements Pay & Benefits per visit, depending on visit type Mileage reimbursement Flexible scheduling options Full-time and part-time opportunities available Health, dental, and vision insurance for full-time employees and eligible family members Company-paid life insurance for full-time employees 401(k) available for both part-time and full-time employees Qualifications Active Arizona Physical Therapist Assistant license Valid driver's license Reliable transportation Ability to travel throughout assigned territory Strong clinical, communication, and documentation skills Preferred Home health experience Residence in Phoenix or the East Valley, including Chandler, Gilbert, Mesa, or nearby communities You'll Be a Great Fit If You Value patient-centered, compassionate care Work well independently while staying connected with the team Communicate clearly with patients, families, and colleagues Stay organized and complete documentation in a timely manner Want to make a meaningful impact in your community Why First Choice Home Health Established, respected agency with over 30 years of service Supportive leadership and team culture Flexibility and autonomy in your schedule Opportunity to build meaningful patient relationships A company that values quality care and doing things the right way If you are a Physical Therapist looking for a flexible and rewarding home health opportunity, we would love to hear from you.
