Kern River Transitional Care Center is seeking a motivated and compassionate Physical Therapist Assistant (PTA) to join our dynamic rehabilitation team. If you are passionate about helping residents regain mobility and independence, we want to hear from you! General Purpose The Physical Therapist Assistant (PTA) assists the Physical Therapist in providing direct resident care and implementing treatment programs designed to improve mobility, strength, and overall functional independence. Essential Duties Provide treatments to residents as directed by the Physical Therapist Complete timely, accurate, and compliant documentation per regulatory and facility standards Maintain a safe and effective treatment environment following infection control and safety protocols Communicate regularly with supervising Physical Therapist regarding resident progress and care plans Participate in care plan meetings, rehabilitation meetings, and resident care conferences Assist with training and support of Restorative Aides Educate residents, families, and caregivers on exercise programs, safety, and fall prevention Recommend and assist with ordering durable medical equipment to support resident independence Maintain confidentiality of resident and facility information Report equipment issues and ensure proper maintenance Contribute to quality assurance and performance improvement initiatives Provide in-service education to staff as needed Foster positive relationships with residents, families, and interdisciplinary team members
Qualifications Education & Experience:
Associate Degree from an accredited Physical Therapist Assistant program Experience in skilled nursing or rehabilitation setting preferred Experience with EHR systems such as Net Health or PointClickCare preferred
Licenses & Certifications:
Current PTA license in the state of California Must maintain license in good standing CPR certification (as required)
Skills & Abilities:
Strong communication and interpersonal skills Ability to follow treatment plans and clinical instructions Ability to work effectively in a team environment Strong documentation and organizational skills Physical Demands Frequent standing, walking, and assisting residents Ability to lift, push, and pull residents and equipment (up to 100 pounds with assistance) Frequent bending, stooping, and reaching Ability to safely assist with transfers and mobility tasks Work Environment This position operates in a healthcare setting with a moderate noise level.
