Pay:

PM No Call Service Area:

Burbank Must have at least 1 year of Home Health experience within the last 3 years. HCHB charting experience is preferred but not required. Responsibilities Assist licensed physical therapists in implementing comprehensive treatment plans tailored to individual patient needs. Perform therapeutic interventions such as trigger point therapy, mobility exercises, and manual therapy techniques to reduce pain and improve function. Monitor patient progress regularly, documenting responses to treatment and adjusting care plans accordingly. Educate patients and their caregivers on proper body mechanics, exercises, and pain management strategies to support ongoing recovery at home. Conduct assessments related to physiology, anatomy, and medical terminology to inform treatment approaches. Support discharge planning by preparing patients for continued care or transition to outpatient or skilled nursing facilities. Maintain accurate and detailed documentation of all treatments provided in compliance with healthcare regulations. Experience Proven experience working as a Physical Therapist Assistant in inpatient settings, skilled nursing facilities, outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practices. Hands-on experience with physiotherapy techniques including trigger point therapy and orthopedic rehabilitation. Familiarity with geriatrics, pediatrics, orthopedics, and chronic condition management within home health or clinical settings. Prior involvement in discharge planning processes and collaboration with multidisciplinary teams. Skilled nursing facility experience is highly preferred for understanding complex patient needs across diverse populations. Demonstrated ability to adapt treatment strategies for acute care situations and outpatient therapies while maintaining a positive attitude focused on patient outcomes.
