Physical Therapist and Physical Therapist Assistant

Duties Conduct initial and ongoing physical therapy evaluations in the patient's home Develop and implement individualized treatment plans based on patient needs and physician orders Improve patient mobility, strength, balance, coordination, and pain management Provide gait training, transfer training, and fall prevention education Educate patients and caregivers on exercises, safety techniques, and use of assistive devices Monitor patient progress and adjust treatment plans as needed Complete all required documentation accurately and in a timely manner Collaborate with physicians, nurses, and other healthcare professionals to coordinate care Ensure compliance with Medicare guidelines and agency policies Participate in discharge planning and patient outcomes evaluation

Qualifications:

Active Physical Therapist (PT and PTA) license in the state of practice Graduate of an accredited physical therapy program Minimum of 1 year of clinical experience (home health experience preferred) Strong clinical assessment and decision-making skills Excellent communication and interpersonal skills Ability to work independently and manage time effectively Valid driver's license and reliable transportation

Pay:

$100.00 - $110.00 per hour
