Willow Creek Health Care Center is seeking a motivated and dedicated Per Diem Physical Therapist Assistant (PTA) to support our therapy team in delivering high-quality rehabilitation services to our residents. General Purpose The Staff Physical Therapist Assistant assists the Physical Therapist with resident-related activities and provides direct resident care in accordance with established plans of care, regulatory guidelines, and facility standards. Essential Duties Provide treatment to residents as directed by the Physical Therapist. Complete timely, accurate, and compliant documentation per facility, state, and payer requirements. Maintain a safe and clean treatment environment following infection control and safety protocols. Participate in care plan meetings and rehabilitation team conferences as needed. Remain compliant with California regulations, including Physical Rehabilitation Manual Article 4 §1398.44. Assist with training and support of Restorative Aides. Communicate regularly with supervising PT regarding resident progress, goals, and discharge planning. Educate residents, families, and caregivers on therapy programs, safety techniques, and fall prevention. Recommend and assist with procurement of adaptive equipment (e.g., walkers, canes, grab bars). Report equipment issues and ensure proper maintenance. Participate in quality assurance and performance improvement initiatives. Maintain strict confidentiality of resident and facility information. Foster positive relationships with residents, families, and interdisciplinary team members. Qualifications Education & Experience Associate Degree from an accredited PTA program Experience in skilled nursing preferred Proficiency with documentation systems such as Net Health and PointClickCare Certificates & Licenses Current PTA license in California (in good standing) CPR certification (if required) Meets continuing education requirements per state guidelines Skills & Abilities Strong communication and interpersonal skills Ability to read and interpret clinical and technical documents Basic math and problem-solving skills Ability to follow written and verbal instructions Physical Demands Frequent standing and walking; occasional sitting, bending, and lifting Ability to lift/move residents and equipment (up to 100 lbs with assistance) Must be able to communicate effectively and respond to sensory cues Capable of assisting with resident mobility and transfers Work Environment Skilled nursing facility setting Low to moderate noise level Fast-paced, team-oriented environment focused on resident care Equal Employment Opportunity (EEO) Statement Willow Creek Health Care Center is an equal opportunity employer.
