Physical Therapist/Physical therapist Assistant (Home Health)

We are seeking Physical Therapists/Assistants to provide in-home therapy services to patients in the Fresno, CA . The ideal candidate will have a passion for helping others regain independence, excellent communication skills, and the ability to work independently while collaborating with an interdisciplinary team.

Responsibilities:

Conduct initial evaluations and develop individualized care plans based on patient needs. Provide therapeutic interventions focused on improving daily living skills, mobility, and safety in the home environment. Educate patients and caregivers on home exercise programs, adaptive equipment, and strategies to enhance independence. Document all visits, progress notes, and care updates accurately and in compliance with home health regulations. Collaborate with nurses, and other disciplines to ensure coordinated and effective care. Maintain accurate documentation in compliance with agency and regulatory requirements. Monitor progress and adjust care plans as needed to achieve optimal outcomes.

Requirements/Qualifications:

Physical Therapist/Physical therapist Assistant License Active Driver's License CPR Certification Liability Insurance Auto Insurance Experience in Home Health (Preferred) This is an Independent Contractor position, CA Business License preferred.
