Job Description

Overview Join our dynamic healthcare team as a Home Health Physical Therapist Assistant, where you will play a vital role in delivering exceptional patient care within the comfort of patients' homes. This rewarding position offers the opportunity to work closely with licensed physical therapists to develop and implement personalized treatment plans that promote recovery, mobility, and independence. Duties Assist licensed physical therapists in evaluating patients' needs and developing tailored care plans focused on rehabilitation goals. Implement therapeutic interventions such as trigger point therapy, manual therapy, and mobility exercises to improve patient function. Monitor and document patient progress accurately, adjusting treatment approaches as directed by supervising therapists. Educate patients and their families on exercises, safety precautions, and discharge instructions to support ongoing recovery at home. Conduct home safety assessments and collaborate with the healthcare team on discharge planning to ensure seamless transitions of care. Maintain detailed records of treatments provided, patient responses, and any changes in condition in compliance with healthcare regulations. Participate in interdisciplinary team meetings to coordinate care for patients receiving inpatient, outpatient, or skilled nursing facility services. Skills Strong knowledge of physiology, anatomy, and medical terminology essential for effective patient assessment and treatment planning. Experience working with diverse populations such as geriatrics, pediatrics, orthopedics, and those recovering from acute or chronic conditions preferred. We welcome new grads and provide mentorship. Ability to develop comprehensive care plans that address individual patient needs and goals. Excellent communication skills for educating patients and collaborating with Physical therapist in charge. Pay:
$124,800.00 - $228,800.00 per year
