Job Overview Join our dynamic healthcare team as a Home Health Physical Therapist Assistant on a 13-week contract, where you will play a vital role in delivering exceptional patient-centered care in the comfort of patients' homes. Pay:
$43-$46/hr plus .54 a mile for patient related travel Local and Travel Welcome! Pomona, CA 13
Week Contract Schedule:
Monday-Friday 8 AM-5
PM No Call Service Area:
Yorba Linda, Orange, Anaheim Hills and Santa Ana Requirements Must have at least 1 year of Home Health experience within the last 3 years. HCHB charting experience is preferred but not required. Responsibilities Assist licensed physical therapists in evaluating patient needs and developing personalized treatment plans focused on mobility, strength, and functional independence. Deliver skilled physiotherapy interventions such as therapeutic exercises, manual therapy techniques including trigger point therapy, and modalities to improve patient outcomes. Monitor patient progress regularly, document responses to treatments, and communicate updates to supervising therapists for ongoing care adjustments. Support patients across various settings including inpatient facilities, outpatient clinics, skilled nursing facilities, and home health environments. Educate patients and caregivers on physiotherapy exercises, safety measures, discharge planning, and injury prevention strategies. Maintain accurate and detailed documentation of treatments provided, patient responses, and progress notes in compliance with healthcare regulations. Collaborate with interdisciplinary teams to coordinate care plans that align with patients' goals and medical requirements. Experience Proven experience as a Physical Therapist Assistant working in home health settings or outpatient clinics. Familiarity with inpatient care, skilled nursing facilities, acute care environments, and outpatient physiotherapy practices. Knowledge of physiology, anatomy, medical terminology, orthopedics, geriatrics, pediatrics, and rehabilitation techniques such as trigger point therapy. Experience developing or contributing to care plans for diverse populations including pediatric patients and geriatric individuals. Strong understanding of discharge planning processes and outpatient treatment protocols. Prior involvement in physiotherapy treatments for orthopedic injuries or chronic conditions within outpatient or clinic settings is highly desirable. Ability to effectively communicate complex medical concepts clearly to patients of all ages while demonstrating compassion and professionalism.
