Responsibilities:

Direct Care:

Implement individualized therapy plans of care established by the supervising Physical Therapist.

Rehabilitation:

Perform gait training, balance exercises, transfers, and therapeutic activities designed to reduce fall risks and relieve pain.

Patient Education:

Train patients, family members, and caregivers on safely performing exercises and utilizing assistive devices like walkers or canes.

Monitoring & Reporting:

Observe patient progress and document their responses to treatments, reporting any notable changes in condition to the supervising PT and interdisciplinary team.

Documentation:

Complete and submit accurate clinical notes in a timely manner according to state regulations and agency standards.

Requirements:

Valid CA PTA License BLS Two Years experience in healthcare setting, one year in home care or community health Valid Driver's License and Auto insurance Proficient with Microsoft Word and Excel. High attention to detail. Able to work independently, take ownership and responsibility. Strong Professional ethics, discretion and judgement.
