In this energetic position, you will work closely with licensed physical therapists to deliver exceptional care and rehabilitation services. Your expertise will help patients regain mobility, improve their quality of life, and achieve their wellness goals. If you're passionate about patient care and eager to make a difference, we want to hear from you! What you'll do Assist in implementing individualized care plans developed by physical therapists. Provide hands-on patient care, including trigger point therapy and physiotherapy techniques. Educate patients on exercises and activities to promote recovery and prevent injury. Document patient progress and communicate effectively with the healthcare team. Support discharge planning by preparing patients for a smooth transition from inpatient to outpatient settings. Collaborate with multidisciplinary teams in skilled nursing facilities, home health environments, and acute care settings. Engage with diverse populations including geriatrics, pediatrics, and orthopedic patients. Basic qualifications Strong knowledge of anatomy and physiology. Familiarity with medical terminology relevant to physical therapy practices. Ability to work effectively in both inpatient and outpatient clinic environments. Preferred qualifications Experience in skilled nursing facilities or home health care. Background in discharge planning processes. Previous work experience with acute care or outpatient rehabilitation settings.
