In this role, you will deliver high-quality, patient-centered care focused on improving mobility, functional independence, and overall quality of life for primarily geriatric residents. This position offers flexible scheduling and the opportunity to make a meaningful impact through skilled therapy services in a supportive, community-based setting.

Responsibilities:

Provide outpatient physical therapy services to residents within assisted living facilities Provide therapeutic interventions including therapeutic exercise, functional training, and manual techniques Educate patients and caregivers on safety, fall prevention, and home exercise programs Collaborate with facility staff and interdisciplinary teams to support optimal patient outcomes Complete timely and accurate documentation Qualifications / Experience Active California PT or PTA license Strong understanding of geriatric rehabilitation and functional mobility training Ability to work independently in a facility-based outpatient setting Additional Information Per diem / flexible schedule
