We are seeking a dedicated and compassionate Licensed Physical Therapist Assistant to join our team. In this role, you will work under the supervision of licensed physical therapists to provide high-quality patient care in our skilled nursing facility. Your primary focus will be to assist patients in their rehabilitation journey, implementing care plans and therapeutic interventions that promote recovery and enhance mobility. Responsibilities Assist licensed physical therapists in developing and implementing individualized treatment plans for patients. Provide direct patient care, including administering therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities as prescribed. Monitor patient progress and report any changes or concerns to the supervising therapist. Participate in discharge planning by helping to prepare patients for a safe transition from therapy to home or other care settings. Educate patients and their families on exercises, home care strategies, and the importance of adherence to the treatment plan. Maintain accurate documentation of patient treatments, progress notes, and care plans in compliance with facility policies. Collaborate with other healthcare professionals to ensure comprehensive patient care.
