Physical Therapy Assistant (PTA) Visiting Nursing Services of Southern CT Hamden, CT Job Details Per diem 2 hours ago Qualifications Discharge planning Acute care Licensed Physical Therapist Assistant Orthopedics Geriatrics Full Job Description Job Summary We are seeking a dedicated and skilled Physical Therapy Assistant (PTA) to work per diem in the Shoreline area. As a PTA, you will work closely with licensed physical therapists to implement treatment plans and assist in patient recovery. Responsibilities Assist licensed physical therapists in developing and implementing individualized care plans for patients for home health services. Monitor patient progress, document responses to treatments, and communicate updates to the supervising therapist to adjust care plans accordingly. Educate patients and their families on physical therapy exercises, mobility techniques, and injury prevention strategies tailored to their specific needs. Collaborate with interdisciplinary teams to deliver comprehensive patient care focused on safety, comfort, and recovery goals. Qualifications Current Connecticut Physical Therapy Assistant (PTA) license is required. Home Care Experience Familiarity with discharge planning processes and documentation standards. Experience working with diverse populations including geriatrics and patients recovering from acute care or orthopedic procedures. Ability to work effectively within multidisciplinary team in a fast-pace environment.
