Physical Therapist Assistant - Home Health ELEvATE Home Rehab, Inc.

Mount Dora, Apopka, FL. We are seeking a dedicated and skilled Physical Therapist Assistant to join our healthcare team. The ideal candidate will work under the supervision of licensed physical therapists to provide high-quality patient care in various settings, including outpatient clinics, skilled nursing facilities, and home health environments. This role requires a strong understanding of physiotherapy principles, anatomy, and medical terminology, as well as the ability to develop and implement effective care plans tailored to individual patient needs. Responsibilities Assist physical therapists in implementing treatment plans for patients with various conditions, including orthopedics, geriatrics, pediatrics, and acute care. Perform therapeutic techniques to alleviate pain and improve mobility. Monitor patient progress and report changes to the supervising physical therapist. Educate patients and their families on exercises, techniques, and home care strategies to enhance recovery. Maintain accurate and timely documentation of patient treatments, progress notes, and discharge planning. Collaborate with healthcare professionals to ensure comprehensive patient care in inpatient and outpatient settings. Participate in the development of individualized care plans based on patient assessments. Ensure compliance with safety protocols and maintain a clean working environment. Qualifications Associate degree in Physical Therapy Assisting or related field from an accredited program. Knowledge of physiology, anatomy, medical terminology, and physiotherapy practices. Experience in skilled nursing facilities or home health settings is preferred. Familiarity with discharge planning processes for effective patient transitions. Strong interpersonal skills with a focus on patient care and communication. Ability to work collaboratively within a multidisciplinary team environment. Previous experience in outpatient clinics or acute care settings is advantageous. Certification or licensure as required by state regulations.
