The ideal candidate will possess a strong background in physical therapy, with experience in home community care and outpatient settings. This role involves providing exceptional patient care, developing treatment plans, and facilitating discharge planning for patients recovering from various conditions, including orthopedic issues. The Physical Therapist will work collaboratively with a multidisciplinary team to ensure optimal patient outcomes. Responsibilities Monitor anf treat patients' physical abilities and limitations. Follow individualized treatment plans based on patient needs and goals. Provide therapeutic interventions to improve mobility, strength, and overall function. Provide education to patients and their families regarding treatment plans and home exercises. Collaborate with PT and other healthcare professionals to coordinate patient care and discharge planning. Maintain accurate documentation of patient progress and treatment outcomes. Stay updated on best practices in physical therapy and participate in ongoing professional development. Ensure compliance with all relevant regulations and standards in home health and hospital settings. Experience Proven experience in physical therapy, in home health. Strong knowledge of anatomy and physiology as it relates to rehabilitation practices. Familiarity with orthopedic conditions and treatment methodologies. Excellent interpersonal skills for effective communication with patients, families, and healthcare teams. Ability to work independently as well as part of a collaborative team.
