• Collaborate to provide management and delivery of patient care plans
• Meet critical deadlines for charting
• Self-manage time and schedule effectively Travel
• Ability to spend approximately 20% of the day driving to/from patient locations Schedule Monday - Friday, 8:00a - 5:00p (no weekends or holidays) Qualifications Required Associate degree or higher 1+ years of experience in ANY of the following: Home Health Care Home Health 1+ years of experience in ANY of the following: Electronic Medical Record (EMR) Practice Partner EMR EHR Implementation Valid Regular Driver License (DL) in Pennsylvania (PA) Valid Physical Therapy Assistant in Illinois (IL) Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months?
