Job Description
Join a thriving team as a passionate Physical Therapist, specializing in home health care in the lively community of Colorado. This full-time role is perfect for those who are enthusiastic about delivering high-quality, patient-focused therapy right in the comfort of patients' homes.
Key Responsibilities:
Perform comprehensive physical assessments and analyze medical histories to evaluate patients' conditions accurately. Create and implement tailored treatment plans that address individual rehabilitation goals and patient needs. Conduct therapeutic exercises and training programs to improve mobility and enhance overall functionality. Educate patients and their families about injury prevention, wellness strategies, and the recovery journey. Monitor and document patients' progress, adjusting treatment plans as necessary to achieve rehabilitation goals. Collaborate with a multidisciplinary healthcare team to ensure holistic patient care. Maintain meticulous patient records and documentation, complying with regulatory standards. If you bring the following, we'd love to connect: Skilled in implementing individualized care plans and communicating patient findings, progress, and changes effectively to the Physical Therapist and treating physician Experienced in educating patients and families on home safety protocols and proper medical equipment use Required Certifications & Licensures Associate's degree from an accredited Physical Therapy program Active and valid license, registration, or permit to practice in the state of agency operation Current CPR certification, maintained continuously throughout employment in a clinical role Valid driver's license with the ability to travel across all business locations as neededSimilar jobs in Lombard, IL
