Responsibilities / What
You'll Do Lead exercises and training programs to improve patient mobility and daily function. Teach patients and families about safety at home, wellness, and how to use medical equipment. Track and chart patient progress, modifying treatments as needed to meet goals. Report changes in patient conditions and progress to the Physical Therapist and physician. Partner with a multidisciplinary healthcare team to provide complete patient care. Keep detailed and accurate medical records that meet regulatory standards.
Qualifications / Who
are we looking for Associate's degree from an accredited Physical Therapy program. Active, valid license or registration to practice in the state. Current, continuously maintained CPR certification. Valid driver's license and the ability to travel to required locations. Experienced in explaining patient progress and teaching home safety protocols.
