Based on the north side of Evansville, our home health and rehabilitation services support patients in Vanderburgh, Warrick, Dubois, Gibson, Perry, Spencer, Pike, and Posey counties. In this role, you'll provide PTA services tailored to each patient's treatment plan in line with our mission to deliver compassionate, high-quality care.

Deliver individualized therapy services as prescribed in treatment plans. Implement and monitor therapy procedures to help patients achieve their goals.
