Job Description

Baker-Katz is a 55-bed skilled nursing facility located on the Haverhill/Groveland line. We are looking for a motivated, self-directed individual who is looking to join our team!
POSITION PURPOSE
Administers Physical Therapy procedures upon medical prescription in accordance with Rehabilitation Department policy and under the supervision of a Physical Therapist (PT).
EDUCATIONAL REQUIREMENTS
Associate's Degree in Physical Therapy from an accredited college or university. Applicant must register and pass the PTA licensure exam within six months of hire date.
EXPERIENCE REQUIRED
Two years of PTA experience in a related setting is preferred.
PROFESSIONAL LICENSURE AND CERTIFICATION REQUIRED
Currently licensed to practice as a Physical Therapy Assistant by the state of Massachusetts. APTA membership is encouraged.
Job duties include:
Implements the outlined treatment program after an evaluation and program plan has been established by the Physical Therapist (P.T.). Reports progress limitations, response to treatment and/or other aspects of resident treatment to the P.T. Identifies need for re-evaluations, goal and plan revisions by P.T., and assists with discharge and follow-up recommendations. Assists with assessment duties as assigned by the P.T., such as home assessments for discharge planning and equipment needs. Documents pertinent information in a clear and precise manner in accordance with regulatory, licensing, payor, and accrediting requirements. Attends/assists in rehabilitation team meetings, resident/family team meetings, home assessments, staff development and inservices, (as assigned). May represent the rehab department at facility committee meetings. Is responsible for care, usage of physical therapy equipment, space and supplies. Advises P.T. supervisor and Rehab Director of supply needs, equipment repairs, etc. Plans daily schedule to maintain the expected standard of productivity; coordinates schedule with the other therapy disciplines to maximize team effectiveness. Provides the necessary accurate billing information regularly and as requested in a timely manner to the Rehab Director. Provides for staff training, inservice and orientation as assigned. Assists in family training, discharge planning, and follow-up recommendations.
Pay:
$35.00 - $43.00 per hour
