Job Description

Job Summary Join our dynamic healthcare team as a Physical Therapist Assistant specializing in skilled nursing facilities (SNF). In this vital role, you will collaborate closely with licensed physical therapists to deliver exceptional patient care, focusing on restoring mobility, reducing pain, and enhancing quality of life for diverse patient populations. Your expertise will support inpatient rehabilitation, outpatient therapy, home health visits, and specialized programs such as orthopedics, geriatrics, pediatrics, and discharge planning. This position offers consistent weekly pay and the opportunity to make a meaningful impact in a fast-paced, supportive environment. Responsibilities Assist licensed physical therapists in developing and implementing personalized care plans for patients across inpatient and outpatient settings. Conduct therapeutic exercises, trigger point therapy, and mobility activities tailored to individual patient needs. Monitor patient progress and document responses to treatment interventions accurately and thoroughly. Support discharge planning by preparing patients for transition to home or other care settings through education and follow-up instructions. Provide compassionate patient care by ensuring safety, comfort, and engagement during therapy sessions. Maintain detailed records of treatments administered, patient responses, and any changes in condition according to facility protocols. Collaborate with interdisciplinary teams—including nurses, physicians, and caregivers—to optimize patient outcomes and ensure seamless continuity of care. Requirements Valid certification as a Physical Therapist Assistant with current licensure or registration as required by state law. Proven experience working in skilled nursing facilities (SNF), inpatient rehabilitation units, or outpatient clinics. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy. Familiarity with diverse therapeutic settings such as home health visits, acute care environments, orthopedics, geriatrics, pediatrics, and outpatient clinics. Ability to develop and follow detailed care plans while adapting interventions based on patient progress. Excellent communication skills to effectively educate patients and coordinate with healthcare teams. $1,740.00 - $1,840.00 per week
