Per Diem Join our team at Allegria Village, where compassionate care and excellence come together. We are currently seeking a dedicated Per Diem Physical Therapist Assistant (PTA) to join our Skilled Nursing Facility team. This flexible, as-needed position is ideal for an individual who is passionate about helping residents achieve their highest level of function and independence. Key Responsibilities Provide direct patient treatment under the supervision of a licensed Physical Therapist, following established plans of care Assist in the implementation and modification of treatment plans to promote optimal functional outcomes Monitor and document patient progress, reporting changes in condition to the supervising Physical Therapist Instruct patients, families, and caregivers in exercise programs and safe mobility techniques Maintain accurate, timely, and compliant documentation of care and treatment provided Adhere to all facility policies, including quality improvement, safety, and infection control standards Communicate effectively with the interdisciplinary care team, ensuring continuity and quality of care Support a positive, team-oriented work environment and contribute to patient satisfaction Participate in ongoing professional development and in-service training as required Perform other related duties as assigned Education & Certification Requirements Graduate of an accredited Physical Therapist Assistant program Current state license as a Physical Therapist Assistant in good standing Qualifications Knowledge and skills necessary to provide care within a Skilled Nursing Facility (SNF) environment Previous SNF experience preferred Understanding of applicable state, federal, and professional standards Strong communication, organization, and time management abilities Compassionate, patient-centered approach with a strong sense of empathy Excellent teamwork and interpersonal skills Positive and professional demeanor Physical Requirements The physical demands described are representative of those necessary to perform the essential functions of this position. Reasonable accommodations may be made for individuals with disabilities. Ability to lift at least 40 lbs. Frequent walking, standing, pushing, pulling, reaching, crouching, stooping, bending, and stretching Ability to assist with patient transfers and perform necessary documentation tasks
