Schedule:

Typical hours are Monday-Friday, 8:30 AM-5:00 PM , with an estimated 4-5 patient visits per day —and the flexibility to adjust your visit times to fit your preferences and patient schedules. What You'll Do Provide in-home physical therapy treatments under the plan of care and supervision of a Physical Therapist (PT) . Implement treatment plans to help patients improve mobility, strength, balance, endurance, transfers, gait, and overall functional independence. Educate patients and caregivers on safe mobility techniques, fall prevention, home exercise programs, assistive device use, and safety strategies. Communicate patient progress, concerns, and outcomes to the supervising PT and care team. Collaborate with nurses, OTs, STs, aides, and other team members to ensure coordinated, patient-centered care. Complete accurate, timely documentation in compliance with home health standards. What We're Looking For Licensed Physical Therapist Assistant (PTA) in the state of Michigan. Home health experience preferred, but not required. Strong clinical skills and a commitment to patient-centered care. Reliable transportation and willingness to travel to patients' homes throughout our service area, approximately a 50-mile radius from Gaylord . Positive, team-oriented attitude—we support each other and do what's best for patients.
