About Us:
AgeWise Mobility is a locally owned physical therapy practice focused on improving mobility, reducing pain, and preventing falls in older adults. We work closely with seniors using evidence based approach for aging in place. Duties Assist licensed physical therapists in implementing treatment plans tailored to individual patient needs, including exercises and manual therapy techniques such as trigger point therapy. Support patient care by monitoring progress, providing encouragement, and ensuring safety during therapy sessions across inpatient, outpatient, home health, or skilled nursing settings. Prepare treatment areas and equipment for therapy sessions, maintaining cleanliness and organization to ensure optimal patient safety and comfort. Educate patients and their families on physiotherapy exercises, discharge plans, and home care strategies to promote ongoing recovery outside of clinical settings. Document patient responses, progress notes, and treatment details accurately in compliance with healthcare regulations and facility protocols. Collaborate with interdisciplinary teams on discharge planning and care coordination to facilitate smooth transitions between care environments. Stay current with industry best practices by participating in ongoing training related to physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, pediatrics, and acute care.
Qualifications:
Graduate of an accredited Physical Therapist Assistant program Current Michigan PTA license (or eligible for licensure) Experience with the geriatric population strongly preferred Experience in outpatient and/or long-term care (LTC) settings recommended Strong interpersonal and communication skills Compassionate, patient-centered approach to care

Pay:
Negotiable, based on experience Experience Proven experience supporting physical therapy teams in inpatient or outpatient settings within skilled nursing facilities or clinics. Hands-on experience with physical therapy modalities and other manual techniques. Knowledge of human physiology, anatomy, medical terminology, and the fundamentals of physical therapy practices. Familiarity with care plans for diverse populations such as geriatrics, pediatrics, orthopedics, or patients recovering from acute injuries or surgeries. Strong understanding of discharge planning processes and patient education strategies to support long-term recovery goals. Demonstrated ability to work effectively within multidisciplinary teams while maintaining a compassionate approach focused on patient-centered care.
