Job Summary We are seeking a motivated and compassionate Physical Therapist or Physical Therapy Assistant to join our dynamic healthcare team. In this role, you will develop and implement personalized care plans to improve mobility, reduce pain, and enhance quality of life for our residents. You will work in inpatient, outpatient, home health, and community care settings, utilizing your physiotherapy expertise to make a meaningful difference every day. You will be contracted through Concept Rehab, but all hours will be performed in our Adrian facility Responsibilities Assess residents' physical conditions by reviewing medical histories and conducting comprehensive evaluations. Design and execute tailored treatment plans focusing on rehabilitation goals, including discharge planning and care coordination. Provide hands-on therapy such as trigger point therapy, and geriatrics, to promote recovery and mobility. Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes in inpatient, outpatient, or home health environments. Collaborate with interdisciplinary teams to ensure seamless care delivery across hospital, clinic, and community settings. Document patient care activities accurately using electronic medical record (EMR) systems and maintain detailed treatment records. Educate patients and their families on physiotherapy techniques, injury prevention, and home exercise programs. Qualifications Valid licensure or certification as a Physical Therapist or Physical Therapy Assistant in accordance with state regulations. Strong knowledge of physiology, anatomy, medical terminology, and rehabilitation techniques. Experience in acute care, hospital settings, outpatient clinics, home health environments, or Long Term Care is highly preferred. Familiarity with discharge planning processes and developing comprehensive care plans. Proven ability to work effectively with diverse populations including geriatrics and pediatrics. Excellent communication skills to educate patients and collaborate with healthcare teams. Prior experience with EMR systems and documentation standards is advantageous.
