Job Description
Citizens Memorial Hospital (CMH) is hiring a licensed Physical Therapy Assistant for our Home Health Department. CMH is made up of 34 primary and specialty clinics, 6 long-term care facilities and 1 residential care facility. Offering acute care, emergency services, ambulatory care, pharmacies, walk-in clinics, surgeries, addiction recovery, rehab therapies and more, CMH has its community covered!
About the department:
The goal of the Home Health Department is to decrease the number of hospital days and hospital visits per patient by providing services directly to patients in their homes. Individualized plans of care are developed specializing services to meet the needs of each patient. Patients, family members, and caregivers are provided with instructions and assistance to learn new procedures, prevent complications, administer medications, control pain, and more.A typical day will include:
Traveling to the homes of patients to provide therapies aimed at improving mobility and recovery. Follows plans of care provided by therapists and other providers Provides patients with directions on the use of rehabilitative devices and equipment Reports patients' progress toward goals Updates patients' records with relevant and timely information This position is: Full-time, Monday - Friday, 8am-5pm, and covers a designated service area.Job Requirements:
Education:
Completion of an accredited Physical Therapy Assistant program with anAssociates Degree Experience:
New graduates welcome; 1 year of experience preferredSkills:
Patient careLicensure/Certification:
Current Missouri Physical Therapy Assistant Certificate Physical:
Prolonged periods of standing and walking, ability to lift, push, and pull up to 50 lbs. with patient and/or equipment assistance, ability to carry up to 35 lbs.Benefits Include:
