Pay Range:

Educational Assistance Program We're Looking For:
Graduate of an approved Physical Therapy Assistant (PTA) program (required). Current state licensure as a PTA (required). Maintain current state driver's license and auto insurance at all times. Personal vehicle must be well maintained and in safe driving condition. 1-year experience as a licensed PT in a clinical care setting or home health (preferred). Basic Life Support for Healthcare Provider (BLS HCP) from AHA (required).Schedule Details/Additional Information:
Monday toFriday, Days What You'll Do:
Manage daily patient caseload, completing timely assessments, treatments, and documentation aligned to plan of care and physician orders Perform comprehensive assessments (home, telephonic, EMR) and apply clinical judgment to prioritize care needs and evaluate therapy effectiveness Deliver PT interventions including modalities, procedures, therapeutic exercise, and patient/family education per established plan Develop, implement, and update individualized care plans, including goals, discharge planning, and resource utilization Monitor, document, and communicate patient progress, treatment responses, and condition changes to physician, PT, and care team Educate patients and families on deficits, goals, home programs, and safe equipment use to support outcomes and independence Ensure compliance with all regulatory, quality, and documentation standards while supporting collaboration, continuous learning, and evidence-based practice Ready to Take theNext Step:
