$1482.00•$1527.00 The facility serves a primarily geriatric population, focusing on post-acute rehabilitation, strengthening, balance training, and functional mobility in a SNF setting. You can expect a steady caseload of short-term rehab and long-term care residents, with a strong emphasis on personalized care plans and measurable functional outcomes. Typical responsibilities include implementing PT treatment plans under the supervision of a Physical Therapist, performing therapeutic exercises and functional mobility training, documenting patient progress in the facility's EMR, collaborating with nursing and interdisciplinary teams, and participating in care conferences as needed. The environment is supportive, with an emphasis on communication and coordinated care that allows you to focus on safe, effective patient progress. Shifts are standard day shifts, 5 eight-hour days, with an expected 40 hours per week and a guaranteed weekly hour structure that supports consistent scheduling. On-call is not expected, allowing for predictable work-life balance. Patient ratios and caseloads are designed to be manageable while maintaining productivity expectations typical of a SNF. Salisbury itself offers a walkable historic downtown, local restaurants and coffee shops, cultural venues, and nearby parks and green spaces.
