Assist residents with mobility, gait training, range of motion, strengthening, and therapeutic activities to promote independence. Monitor patient responses during therapies, document progress, and communicate notable changes to the supervising PT and care team. Ensure patient safety during transfers, positioning, and functional activities; apply correct assistive devices as prescribed. Prepare treatment spaces and equipment, maintain clean and safe environments, and follow infection prevention guidelines. Document all interventions and outcomes promptly and accurately in the patient record per facility policy.
