Part Time-PRN (8-24Hours Weekly) About Us We are a compassionate, patient-centered hospice and home health organization dedicated to providing high-quality care to individuals and families during some of life's most challenging moments. Our interdisciplinary team works collaboratively to support patients' physical, emotional, and psychosocial needs in the comfort of their homes. As a small, locally owned Hospice & Home Health agency, we pride ourselves on caring for our patients the same way we would care for our own family. Position Summary We are seeking a dedicated and compassionate Physical Therapy Assistant (PTA) to join our hospice and home health team. In this role, you will work under the supervision of a licensed Physical Therapist to help patients improve mobility, manage pain, regain strength, and maintain independence in the comfort of their homes. You will play a vital role in enhancing patients' quality of life through individualized therapy and supportive care. Key Responsibilities Provide physical therapy treatments and exercises as directed by the supervising Physical Therapist Assist patients in improving mobility, balance, strength, endurance, and functional independence Monitor and document patient progress, response to treatment, and changes in condition Educate patients and caregivers on therapeutic exercises, safety precautions, mobility techniques, and use of assistive devices Support patients and families with compassionate care and encouragement throughout treatment Maintain accurate and timely documentation in accordance with organizational and regulatory standards Observe infection control, patient safety, and confidentiality protocols at all times Qualifications Associate degree from an accredited Physical Therapist Assistant program Current state PTA license in good standing CPR certification Minimum of 1-2 years of experience in home health, hospice, rehabilitation, or healthcare preferred Strong interpersonal and communication skills Ability to work independently and collaboratively within an interdisciplinary team Compassionate, patient-focused approach to care Valid driver's license and reliable transportation What We Offer Competitive compensation Flexible scheduling options Supportive team environment PTO/Sick Time starts immediately Retirement match after one (1) year of employment How to Apply Interested candidates are encouraged to submit their resume and a brief cover letter outlining their experience and passion for hospice and home health care.
