Position Summary Masonic Care Community is looking for a full-time motivated, compassionate Physical Therapist Assistant who will support the delivery of high quality care to residents in our short-term rehab as well as long term care within our skilled nursing facility. Under the direct supervision of a licensed PT, the PTA will carry out individualized treatment plans with emphasis on mobility, balance, functional independence, safety and improving overall quality of life. Key Responsibilities Provide physical therapy treatments (individualized, concurrent and group) as established in the plan of care, including therapeutic exercise, therapeutic activity, gait training and functional mobility, balance activities as well as modalities as permitted by NYS practice guidelines. Assist residents in performing therapeutic exercises and functional activities to improve their strength, coordination, mobility and functional activity tolerance. Document resident performance, progress and response to treatment and report any changes and/or recommendations to the supervising physical therapist. Participate in staff meeting, care conferences and discharge planning. Participating in educating residents, families and caregivers for safe transfer/mobility techniques, fall prevention and home exercise programs. Maintain effective, accurate and timely documentation in accordance to facility, state and federal guidelines. Demonstrate proper use of equipment and assist in ensuring a clean, safe and therapeutic environment. Demonstrate professional, ethical and regulatory standards as well as compliance with HIPPS and facility policies. Qualifications Graduate of an accredited PTA program Current state licensure or eligibility for NYS licensure as a PTA Experience in a skilled nursing facility as well as short term rehab and long term care Excellent communication skills, teamwork and resident centered approach to care Strong organizational, time management and skill prioritization in order to work in a fast-paced setting Proficiency with electronic medical record documentation and regulatory compliance requirements Schedule Full time:

M-F 7.50

hr/day May include weekends and/or holiday rotations The base range for this position is $28 - $31 per hour commensurate with work location, experience, skills, certifications, education & prior accomplishments. The position may be eligible for additional compensation not listed here, including but not limited to shift differentials, incentives or bonuses, as applicable.
